New Office Check-in Procedures
At Monarch Dermatology, we strongly value the health and safety of our patients & staff. Therefore, it is with an abundance of caution that we are taking certain necessary precautions due to COVID-19 within our practice. We thank you for your understanding and compliance!
NEW PATIENTS: It will be required to email new patient forms prior to your visit. These forms can be found here on our website. Completed forms should be emailed to: [email protected]
ESTABLISHED PATIENTS: Please inform the office prior to your appointment regarding changes to your insurance, address, phone number, etc.
--Please arrive 5-10 min prior to your scheduled appointment time. Remain in your car and call the office at 732-739-3223 to inform the staff of your arrival. You will then receive a call to come in.
--Patients are to come alone to the office. One caregiver/guardian is allowed for minors and for individuals requiring a caregiver for medical reasons.
--Everyone entering the office will be required to wear a mask. The mask should be worn throughout the visit, except when evaluation of the area under the mask is necessary.
--If you have a fever, cough, shortness of breath, chills, sore throat, or a household member or close contact has been diagnosed with or exposed to COVID-19 in the last 14 days, please call to reschedule your in-office appointment to a telemedicine appointment. All patients will be screened for these symptoms prior to their visit.
--We kindly request that you cancel or reschedule by noon the day prior to your appointment. This allows us to accommodate other patients who need to be seen as we will be limiting the number of patients being scheduled in order to be able to maintain better social distancing and minimize physical interaction.

Telemedicine Visits are Currently Available at Monarch Dermatology
Telemedicine is a virtual audio and video conference between you and your physician. Most smartphones and computers with cameras are capable of accomplishing a telemedicine visit for our practice. See instructions in the following link on how to use each method.

Please be aware that virtual visits will be billed to your medical health insurance just as they would for an in-person office visit. If your insurance has recently changed, please have that information ready to provide to our team members. Depending on your insurance plan, you may have financial obligations such as co-pays, deductibles, and co-insurance just as you would for an in-office visit.
